This document includes information regarding the member's title, address, capital, registration date, degree and field of activity.

In addition, the following statement is included above: "It has been understood from the examination of the file that the company, which is a member of our chamber with the registration number and title written above, is engaged in written business at the written addresses. This document has been prepared and delivered by us to be submitted to the relevant authorities upon the written request of the company."
